Accompanied Service

Need assistance with medical appointments?
L'Chaim Senior Services (LCSS) can help! With our Accompanied Service we pick you up at your home, drive to your medical appointment, stay with you and drive you back home.

Who can use Accompanied Service?
You must be a client of LCSS and be at least 60 years old. A case manager's assessment is required. You must also be able to transfer in and out of a vehicle. Wheelchairs or oxygen tanks cannot be accommodated. No pets allowed in the vehicle.

Where can I go?
The program serves the greater Minneapolis area with some geographic restrictions.

How much does it cost?
The fee is $13/hour with a two-hour minimum. Limited to two trips per month. (Sliding fee scale is available).

The JFCS counseling staff strives to maximize each person’s strengths and skills. Each counselor works to educate and empower people by developing an individualized treatment plan using a variety of therapeutic methods. JFCS maintains the highest ethical and clinical standards of professional practice.

PREPARE/ENRICH is a dynamic program designed to enhance marital growth and revitalize relationships. The JFCS Counseling Department program assists couples of all types - those considering marriage, newly engaged couples, gay/lesbian couples, and those married for years. Through PREPARE/ENRICH, couples can assess their compatibility, learn to resolve differences, handle financial decisions and increase their communication skills. JFCS counselors are skilled in working with diverse couples.

Deikel Family AlterCare Adult Day Program

This adult day service program is a wonderful environment for people who otherwise might be alone and isolated during the day. Adults with memory and/or physical impairments, dementia and developmental disabilities will enjoy a safe, stimulating and supportive environment at the Deikel Family AlterCare Program. It offers respite for the at-home caregiver during the day. Located on the Ackerberg Community Campus in St. Louis Park. Learn more about Sholom Community Alliance.

Occasionally support groups are offered for caregivers of individuals with memory loss, dementia and Alzheimer's disease. If you are interested in participating, please call 952-939-1511.

All AlterCare staff have undergone extensive training on Alzheimer's disease and dementia-related topics to meet licensure requirements mandated by the State of Minnesota.

   

Deikel Transportation Services

Here’s a convenient, reliable way for adults 60 years and older to get to a doctor’s appointment, friend’s house, grocery store and more. Deikel Transportation provides rides for clients of the JFCS L’Chaim Senior Services Program who reside within a defined service area in Hennepin County.

When people don’t know where to turn for help, they can call JFCS. The Information and Resource Connection staff has a thorough knowledge of resources in the Jewish and general communities. Information and Resource Connection staff can help you connect with our services and manage much of the emergency financial assistance available through JFCS.



   

Kosher Meals on Wheels

JFCS provides a nutritious, kosher, mid-day meal, five days per week, delivered to people who are homebound or have difficulty preparing meals. The kosher Meals on Wheels program allows people to maintain their dietary practices. All meals are prepared in the kosher kitchen at Sholom Home West.

The professional staff at JFCS works closely with adults age 60 and older and their families to ensure safe, supported and independent living at home. Participants receive case management and services such as transportation, shopping assistance, housecleaning, kosher Meals on Wheels, medication set-up, bathing, foot care and health monitoring. Mental Health Support Services (MHSS)

Mental Health Support Services works with people who live with serious and persistent mental illness. The program helps people improve connections with family, work and community, while achieving and maintaining health, stability, and independence.

Survivor Support Services

Survivor Support Services is a program of L'Chaim Senior Services (LCSS) that provides service to Jewish victims of Nazi persecution. All the services of LCSS are available to survivors as they encounter the hardships of aging.

In addition to a case manager who can advocate and coordinate services, additional assistance can be provided to survivors and to their family members with the process of applying for restitution and compensation.

The survivors may also receive help accessing national and international organizations, which coordinate other compensation programs.

Survivors may also receive service from other programs of JFCS, such as Counseling, Family Life Education, Volunteer Resources, or be a volunteer.

Staff of the program provide training to the community on a variety of survivor issues, such as teaching staff in long-term care about managing survivor patients.

There is an agency-staffed advisory committee whose members are Holocaust Survivors that provides oversight to the program.

This program receives funds from the Conference on Jewish Material Claims Against Germany for the Emergency Assistance Program for Nazi Victims at the direction of the United States District Court supervising the lawsuit In Re: Holocaust Victims Assets Litigation (Swiss Banks).

With trained volunteers and access to information and referral, this unique program answers the call for many individuals and their families encountering life-altering situations.
